Getting it wrong!

We like to think we get things right.  If we are known for something, we want people to know we do it well.

Sometimes it helps to see what it looks like when things are really, really wrong.  With that in mind, here are a few cases where things went hopelessly wrong.

  1. Nothing ever happened because of one thing acting alone by itself!

    Wouldn’t the following questions be better than “Why?”



    One: What are the conditions, behaviors, actions, and inactions that directly resulted in the harmful effect?

    Two: What are the conditions, behaviors, actions, and inactions that directly resulted in the effect being investigated?

    If you ask, “why”, you usually get only one harmful condition, behavior, action, or inaction, but if you ask the suggested questions you can get all of the conditions, behaviors, actions, and inactions that directly resulted in the effect. Isn’t that better?

    Never accept one singular answer to a question about causation, even if it begins in “why.”

    If you did ask “why”, go back and ask, “What are the other conditions, behaviors, actions, and inactions that directly resulted in the effect being investigated?”
